AYJ comment: Tragic death of 16 year old boy in HMYOI Feltham

Our hearts go out to the family, friends, and all who loved the 16-year-old boy who died at HMYOI Feltham this week. This is a devastating tragedy - the loss of a young life with so much still ahead.

There are concerning variations between the accounts provided by the boys loved ones and the police regarding the circumstances of his death. While we must wait for the official investigation to establish the facts, these conflicting reports are deeply troubling and underscore the need for total transparency.

Regardless of the eventual findings in this specific case, this loss occurs at a time when the broader safety and suitability of Young Offender Institutions (YOIs) are under scrutiny. Chronic understaffing, unsafe conditions, and the frequent use of isolation across the estate has created environments that struggle to meet the basic standards of care required for children. The Ministry of Justice’s own February 2026 policy paper describes current conditions as "dismaying."

The government’s forthcoming youth justice reforms must now be a turning point. It is no longer enough to acknowledge that the current system is failing; the government must set out a time-limited plan for the closure of YOIs.

We cannot wait for further roadmaps or improvement programs while children remain in environments that cannot guarantee their safety and wellbeing. The government has a duty of care to every child in the justice system. The only way to prevent future tragedies is to commit to a clear plan ensuring that custody is used only as a last resort, and that when it is necessary, children are held in rights-respecting and trauma-informed settings.
